I, _______________________ the Parent/Legal Guardian of ___________________ understand that by checking the box below, acknowledge that: 1. Although every effort is made to send campers home with all of their belongings, Praxis is not responsible for any loss of or damage to or personal belongings. 2. My child will be expected to follow all reasonable instructions from the staff of Praxis while participating in the program, and is expected to lead, interact with, and help out the foreign exchange students in our activities. 3. Praxis Learning International Inc. reserves the right to cancel a child’s participation in a camp session if their behaviour is deemed unmanageable or dangerous to themselves or others, in which case they will be sent home immediately 4. I hereby release Praxis, its owners, directors, offices, agents, staff, employees, volunteers, associates and Unity Christian School of and from all manner of actions, claim and demands of whatever nature which result from any loss, injury, damage or expense sustained, arising out of or in any way connected with participation in any program or attendance at a location operated by Praxis Learning International Inc. 5. In the event my child is injured, ill or in need of medical attention, I authorize Praxis staff to seek medical attention on my child’s behalf. In the event my child needs medical care for emergency reasons, I understand that Praxis will attempt to make contact with me. If contact cannot be made on first attempt, I will be deemed to have authorized by Praxis to seek medical attention for my child. 6. I hereby authorize Praxis to take photographs or ask for testimonials from participants throughout the course of their program participation. These photographs may be used during presentations and/or for advertising and promotional purposes on websites, flyers, brochures, etc. By signing this form, I give permission for my child’s photograph, name and/or testimonial to be used in Praxis’ promotional and marketing materials, including but not limited to their website. 7. Release of Liability: I understand there are risks associated with all activities and programs of the Summer Camp . I hereby waive and release any and all claims I may have against Praxis Learning International Inc., its directors, officers, agents, staff, employees, Unity Christian School and volunteers for any claims, loss, damage or injury to my child or my child's property that may occur through the normal course of Praxis Learning International Inc.’s operations including but not limited to any negligent acts or omissions by Praxis Learning International Inc. . I also further agree to indemnify and save harmless Praxis Learning International Inc. for all costs that may be incurred as a result of injury, death, illness or disease, including attorney fees, defense costs incurred in connection with claims of physical, mental or emotional injury or property damage that my child may negligently or intentionally cause to himself/ herself or to any third parties in the course of his/her attendance and/or participation in any activities or programs as part of the Praxis Learning International Inc.’s Summer Camp. 8. I understand that in order to succeed in the Praxis Summer Camps, my child must attend all my classes and participate in all activities. I understand and acknowledge that the program reserves the right to terminate my child’s participation in the Summer Camp, if my child fails to adhere to program regulations including but not limited to the following prohibited activities: driving a car; drinking alcoholic beverages or using illegal drugs; working illegally; becoming involved in any illegal activities; breaking camp rules such as poor conduct; curfew violations; chronic absenteeism. I have read, understand and reviewed the above with my child and acknowledge that my child is expected to abide by these rules, as well as to co-operate with the program administrators, teachers and other students during the camp. 9. I understand that this is the entire agreement between myself and Praxis Learning International Inc., its agents, employees, staff, contractors, and volunteers, which cannot be modified or changed in any way by the representations or statement of any employee or volunteer of Praxis Learning International Inc. or by me. 10. By checking the box below, I have indicated that I have read this waiver document and the and that I understand it completely and agree to be bound by all the terms and conditions contained herein.
